Insurance Waivers - Employees not electing insurance benefits for the 2007-2008 school year can fill out an Insurance Waiver Form which is located on our website. 

Dental Insurance - MetLife is our current carrier and our group number is 303341 and your social security number is your ID number.  Information is available on the MetLife website at www.metlife.com (type in EBC at the company name prompt).  The benefit summary sheet is also on our website.

Blue Cross/Blue Shield PPO and HMO benefit summary sheets are available on our website.

Vacation /Sick/Personal Days - Please note that your available days are posted on your check stub.  The paycheck will not reflect the current month's activities.  Although the check stub will show available vacation (if applicable), sick and personal days; you are ultimately responsible for the accuracy of your own records.  Probationary employees are subject to the terms of the Collective Bargaining Contract regarding available of sick days that are posted on the check stub.  See page 28 of the contract.
