There's a
new form in town! Introducing the new Leave Request Form located
on the Employee Forms link. The new form has been updated to
allow the user to compose directly on the form. Once the form is
completed, it must be emailed to your immediate supervisor. In
addition, you must contact Emma Dabney at
(708) 342-5350. A confirmation of the approved absence will be
sent to you from Melody Morrow.
